Excellent opportunity to drive guidelines for a new Home Infusion program.  This position requires a licensed pharmacist who is familiar with utilization management for infusion therapy.

Our ideal candidate will be able to work independently and will have a background in program/product development and/or driving special projects.

This is a Remote position with 25-40% travel to field.  Travel is in the East coast.

Responsibilities:
  • Researching medical and pharmacy literature to develop, update, and implement clinical criteria and pathways to support the client's new Home Infusion Program, Medical Oncology and Specialty Drug Management Programs. 
  • Collaborate with colleagues (nursing, physicians, operations, analytics and information technology) and external clients to develop new or enhance existing evidenced-based programs that support delivery of effective, safe and affordable medication therapy in the home setting.
  • Conduct literature reviews of medical and pharmacy journals, medical guidelines, and other references to develop medication use policies, guidelines and/or clinical decision support algorithms for clients.
  • Coordinate clinical reviews with clinical experts and other medical specialists
  • Participate in and lead discussions with internal and external partners to present/review developed clinical programs
Requirements
  • MS or Doctorate in Pharmacy
  • Licensed Pharmacist
  • 4+ years of practicing Pharmacist experience working in in home infusion, hospital, specialty pharmacy or related healthcare environment
  • Minimum of 3 years pharmacist experience in medical or drug information and/or clinical policy development for specialty pharmacy therapies including oncology medications.
  • Must be familiar with infusion therapy and specialty drug software.
  • Experience in medication therapy management for patients receiving infusion therapies administered in the home and ambulatory care setting.
  • Knowledgeable in various infusion related therapies, drug delivery systems and related supplies for administration. Infusion therapies include parenteral nutrition, IV antibiotics, pain management, hydration and various IV/infused specialty drug therapies
  • Basic knowledge of pharmacy benefit and or medical benefit drug utilization management programs and compliance standards.
  • Ability to work independently from a home-based setting.
